The Social Prescriber role at Livewell Southwest in Plymouth involves working collaboratively with the FUSE Team to enhance community health and wellbeing. This position focuses on supporting clients with complex needs by signposting them to local services and activities that address their holistic health requirements. Responsibilities include conducting assessments, co-designing support plans, and maintaining communication with healthcare professionals and community organisations.

In addition to facilitating access to resources, the Social Prescriber will monitor and evaluate client outcomes, contribute to public health research, and participate in health promotion activities within the community. The role demands adherence to health and safety policies, confidentiality, and the provision of necessary training, all while fostering strong relationships with various stakeholders to effectively support service users in their journey towards improved wellbeing.
