The role will include supporting patient experience and patient access to the services to meet clinical need. The post holder will Provide clinical leadership to the redesign and implementation of new clinical pathways, transforming services across primary, community and secondary care. Strategically lead the design and implementation of those clinical pathways Work with medically registered clinical pathway leads and draw them into discussions where a medical perspective is required Be a senior member of a working group focusing on the redesign and implementation of new clinical pathways, taking the lead on the tactical implementation and delivery of agreed plans, programmes and projects. Review relevant data and intelligence to draw meaningful conclusions and inform conversations with relevant stakeholders and providers to reduce inequalities in access, experience, and outcomes. Work closely with ICB, Primary Care, and Trust staff, clinical and managerial, to deliver plans in a timely way. Work with partners to identify additional opportunities for service redesign for the benefit of the system and patients to ensure efficient service delivery in the right place by the right person. Explore and promote opportunities to develop digital solutions. Ensure risks, issues and blockages are identified and mitigated as quickly as possible.
