The role is based in the Chesterfield area - 37.5 hours per week. Working collaboratively as part of a sector team to meet the needs of our patients; delivering the highest standards of patient care. We are a team of 200 people dedicated to our patients, working across 3 sectors covering Chesterfield, Clay Cross and Heanor. You need to be passionate about our patients, with a desire to improve our population health and wellbeing by shifting from treatment to prevention. This is an excellent opportunity to join a different kind of GP practice, an innovative, forward thinking training practice with a strong commitment to improving our community's health and addressing inequalities. Acting within your professional boundaries, you will assess, diagnose, and manage patients with a range of acute, non-acute and chronic medical problems. Managing daily surgeries comprising of face-to-face appointments, telephone appointments, telephone triage and undertaking home visits when appropriate. You will be a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, using your skills and experience to establish a diagnosis and management plan. You will clinical assess and manage patients with chronic and ongoing health problems. You will use your individual skills and experience to make a positive contribution to the organisation and the development of a \"one team\" approach to provide high quality patient services within and across primary care.
