Acting within your professional boundaries, you will assess, diagnose, and manage patients with a range of acute, non-acute and chronic medical problems. Managing daily surgeries comprising of face-to-face appointments, telephone appointments, telephone triage and undertaking home visits when appropriate. You will be a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, using your skills and experience to establish a diagnosis and management plan. You will clinical assess and manage patients with chronic and ongoing health problems. You will use your individual skills and experience to make a positive contribution to the organisation and the development of a \"one team\" approach to provide high quality patient services within and across primary care.
