Chesterfield Royal Hospital NHS Foundation Trust is seeking an experienced Advanced Nurse Practitioner to join their multidisciplinary primary care team in Grangewood, Chesterfield. This full-time position involves delivering high-quality patient care by assessing, diagnosing, and managing a variety of acute and chronic medical conditions. The role emphasises effective clinical decision-making and collaboration within a team of clinicians, including GPs, pharmacists, and mental health practitioners, to improve community health and address health inequalities.

The successful candidate will act as a first point of contact for patients, handling face-to-face and telephone appointments, as well as home visits when necessary.
