This role offers you the flexibility to work on days that suits you and the chance to work additional hours. Our work is crucial to the community as we aim to prevent unnecessary hospital submissions and care for people in their hoes with a range of conditions including wide range of conditions including diabetes, wound and end of life care. As this is a community role, it is essential that you have a valid UK driving licence and access to a vehicle for work purposes. Responsibilities include but are not limited to- To assist in providing nursing care as delegated by the Senior Sister/Team Sister within patients homes. To maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients. To deliver care in accordance with organisation policies, procedures and guidelines, according to competencies, as delegated by the Senior Sister/Team Sister To report any changes in condition of the patient to the Senior Sister/Team Sister or in their absence the designated nurse in charge of the caseload.
