Intermediate care supports people to avoid going into hospital or residential care unnecessarily facilitating people to be as independent as possible after a stay in hospital, or a period of not managing within their own home. Our unit provides high quality care to patients requiring rehabilitation, to optimise health before returning to their own home, we work in collaboration with Physiotherapists, Occupational Therapists and Social Workers to do this. Advanced communication skills and the ability to act as an excellent role model are fundamental aspects of this role. Acting as patient advocates when needed, an understanding of mental capacity, DOLS is an important part of this job. The successful candidate will be expected to work alongside the existing Nurse Practitioner / Team Lead, as well as be part of the wider MDT ensuring high quality rehabilitation care is provided, a keen interest in rehabilitation is vital for this role. The right candidate will be enthusiastic, dynamic and able to work under pressure. Awareness of computer systems and electronic patient records is advantageous.
