The Community Mental Health Team (CMHT) is a generic team integrated with Croydon Social Care in working with all adults experiencing severe and enduring mental illness. We are looking for an enthusiastic discharge coordinator that will grow and develop with the locality teams from Nursing, Social work, and OT professional background. This advert is for vacancies in CENTRAL LOCALITY TEAM The Post holder will be a CPN, OT or social worker working as a discharge coordinator in Croydon's newly formed locality teams working directly with care coordinators to identify patients whose care needs are best met in another appropriate service. The post holder will work closely with Care coordinator in identifying any barriers to discharge upon acceptance into the team implementing community care process model, CPA/review meetings. They will work 'hand in hand' with their colleagues in the team, other community teams in and out of the trust, primary care colleagues and the patient's family/carers to facilitate discharge. They will ensure that the estimated discharge date as identified during reviews is progressed where possible and feasible. About us Clinical To take the lead in the discharge planning for patients that no longer require intensive input from the CMHT or are required to be transferred to another team for varied reasons. To ensure comprehensive and accurate patient information collection, from initial referral, through their stay with the team and on to discharge. To use this information to complete transfer and discharge CQUINNs forms accurately to GPs or other CMHTs, and other teams within the CMHT.To support discharge from care coordination by working through any barriers to discharge at the outset of the acceptance into the team.To deliver a range of effective, service user-centred interventions aimed at crisis resolution and promoting recovery.
