The Community Mental Health Team (CMHT) is a generic team integrated with Croydon Social Care in working with all adults experiencing severe and enduring mental illness.

The Post holder will be a CPN, OT or social worker working as a discharge coordinator in Croydon's newly formed locality teams working directly with care coordinators to identify patients whose care needs are best met in another appropriate service.
The post holder will work closely with Care coordinator in identifying any barriers to discharge upon acceptance into the team implementing community care process model, CPA/review meetings. They will work 'hand in hand' with their colleagues in the team, other community teams in and out of the trust, primary care colleagues and the patient's family/carers to facilitate discharge. They will ensure that the estimated discharge date as identified during reviews is progressed where possible and feasible.

To take the lead in the discharge planning for patients that no longer require intensive input from the CMHT or are required to be transferred to another team for varied reasons. To ensure comprehensive and accurate patient information collection, from initial referral, through their stay with the team and on to discharge. To use this information to complete transfer and discharge CQUINNs forms accurately to GPs or other CMHTs, and other teams within the CMHT.To support discharge from care coordination by working through any barriers to discharge at the outset of the acceptance into the team.To deliver a range of effective, service user-centred interventions aimed at crisis resolution and promoting recovery. Supporting service users to adopt recovery strategies that promote their wellbeing, healthy lifestyle, independence and self-careTo undertake thorough risk assessment of all service users in line with policy standards to manage risk safely and effectively to enable effective and safe dischargeTo ensure that discharge plan is carefully and meaningfully planned within the multidisciplinary team, facilitated effect delivered to a high standard and evaluated regularly.To operate within the legal requirements of the Mental Health Act and can exercise independent judgement in the assessment of mental health conditions.
To ensure that there are plans in place for those patients that may have ongoing physical health condition and yet are to be discharge from Mental health services.
Maintain relevant clinical and pharmaceutical knowledge, critically appraise and apply information in practice
Support service users to adopt recovery strategies that promote their wellbeing, healthy lifestyle, independence and self-care.
Actively promote health education that may include providing alternative materials to support service users and carers in making decisions regarding their care and treatment
To provide relevant support to carers and families in relation to their rights as carers and actively support their involvement in the care of the service user.

To receive regular clinical supervision
Maintain and develop own professional knowledge and competence by keeping informed of advances and research pertaining to mental health care.Maintain a safe environment for all staff and visitors, taking reasonable care to avoid injury. Co-operate with the Trust to meet statutory requirements.Provide statements regarding untoward incidents, as requested by line manager.
