The Mental Health Discharge Coordinator role in Croydon, offered by the South London and Maudsley NHS Foundation Trust, is centered on facilitating patient transitions from intensive community mental health services to appropriate alternative care. This position requires collaboration with care coordinators, multidisciplinary teams, and patients' families to identify barriers to discharge, implement effective discharge planning, and ensure comprehensive documentation throughout the patient’s care journey. The post holder, who should have a background in nursing, social work, or occupational therapy, will also conduct risk assessments, deliver recovery-oriented interventions, and actively promote health education among service users and carers.
