South CAMHS is a multi-disciplinary team providing mental health assessment and intervention to young people aged 0-18 presenting with complex mental health difficulties. Our team consists of psychiatry, clinical psychology, systemic therapists, child psychotherapist, clinical social workers, nursing, CAMHS practitioners and support workers. As part of this role you will be working closely with our care coordinators, psychiatry and our Clinical Nurse Specialist to support young people to engage with mental health services. This role is both clinic based as well as involving significant outreach and community based support. We are looking for an enthusiastic and dynamic individual who can work flexibly to support our young people. It is essential for the candidate to have experience of working with individuals with mental health difficulties and an interest in working with young people in particular. As part of this role, you will receive regular supervision and support and will have opportunities to further develop your skills. About us Lead on all referrals to the outreach team, offering consultation, creating clear formulation of work to be undertaken, including a care plan. Create clear working plan around the intervention being offered to CYP, family and clinical support. Appraise the CAMHS Nurse Specialist and MDT of the young persons progress and observations of changes in their mental health condition and ensure these changes are documented on the electronic record. Have a close working relationship with the CAMHS Nurse Specialist, team clinical lead and nominated care coordinators. Regularly meet with the supervisor and manager to ensure the post holder is supported to carry out the demands of the post. Offer regular support and supervision to other support workers within the team. Ensure that all care services provided are offered without prejudice regardless of age, creed, gender and ethnicity. Aware of diverse cultural and ethnic needs and facilitate appropriate support and care. Support clients with activities of daily living, support to attend school and appointments. Accompany the client in attending outpatient, GP and other appointments relevant to current health or social needs as part of the individuals care plan if agreed to do so. "}
