Hertfordshire Partnership University NHS Foundation Trust is seeking a Senior Community Outreach Support Worker for their Specialist Community CAMHS team in South Hertfordshire, specifically based in Watford and Borehamwood. This role involves providing mental health assessment and intervention to young people aged 0-18 with complex mental health issues. The position requires collaboration with care coordinators, psychiatrists, and a Clinical Nurse Specialist to engage young individuals in mental health services, involving both clinic-based and community outreach work. Candidates should have experience in mental health care and a particular interest in working with young people.

The Senior Community Outreach Support Worker will lead referrals to the outreach team, develop care plans, and monitor the progress of young clients, ensuring that changes in their mental health conditions are documented. The role includes supervising other support workers, providing inclusive care that respects diverse cultural backgrounds, and assisting clients with daily living activities, such as attending school and appointments. Regular supervision and opportunities for professional development will be provided to support the individual in this dynamic and flexible role.
