We have vacancies in this exciting service set up within Bedfordshire where you will have the opportunity to work with people living with dementia and other mental health diagnosis in order to reduce distress and increase their quality of life as well as those that care for them. The Dementia Intensive Support Service and Older People's Crisis Team will provide an urgent response to people living with dementia and other mental health conditions in their own homes and 24hour care settings 7 days per week, 365 days per year from the hours of 9am to 8pm. With a 4 hour response time, timely and effective assessment and excellent care planning strategies will be key to helping people to stay in their own homes with reduced levels of distress, reduce 1:1 supervision and prevent unnecessary hospital admissions and transfers of care. If you are passionate about Older People living with Dementia or other Mental Health conditions and are skilled in providing person centred care, we want to hear from you. We are looking for applicants that have the necessary skills and experience and may come from either in patient or community background. The post holder will work as part of a multidisciplinary team based in Beech Close Dunstable and Twinwoods Bedford. The post holder will be expected to visit service users in their homes or in community locations. The post holder will support care home, service users and their family by educating, advising and give practical support to reduce admissions into hospital, reduce one to one care if appropriate. Work will cover Bedfordshire area except Luton and hours of work will be 9am to 8pm, 7 days per week. About us The post holder will be a member of a multi-disciplinary team providing support to clients in care homes with 1:1 care and their carers/ families. The aim of the service is to help individuals with Dementia and other Mental Health Diagnosis to be supported in their place of residence (own home, care/nursing homes, extra care) by the people with whom they are familiar plus the input of the DISS/OP CRHT for as long as clinically appropriate. The key aim of the support worker is to work alongside the multi-disciplinary team to 1. Reduce the number of people relocated to care homes. 2. Reduce numbers admitted to hospital A&E. 3. Reduce out of area placements. 4. Reduce the level of NHS Continuing Healthcare (CHC) one to one support for those assessed as requiring a level of care need because of illness or disability. 5. Training care home staff to better care for residents with Dementia. 6. To help those with Dementia live an increased quality of life. 7. To support older people in a Mental Health Crisis. This role is car drivers who hold full drivers licence only. "}
