City Health Practice Limited provides high quality primary care services within its 3 GP Practices Main duties of the job The post holder will be a fully experienced General Practitioner who will work as a part of a multi-disciplinary team to provide services in general practice to meet the health care needs of those people who are registered at the practice. The post holder will be responsible for ensuring that the highest standard of clinical care is provided for patients across the service. The post holder will initiate clinical assessment including telephone triage, diagnosis, treatment, referral using a range of advanced skills and expert knowledge for patients presenting with undifferentiated and undiagnosed conditions. Doctors must have full GMC registration, Professional Medical qualification, Certificate of prescribed or equivalent experience (JCTGP) and/or A Certificate of Completion of Training and a commitment to ongoing professional development. To provide high quality General Medical Services to patients registered to City Health Practices (CHPL) , identifying health needs and achieve health gain for this population. The post-holder will also contribute to planning and decision making as part of the health care teams.
