The post-holder will contribute to the delivery of patient's individual care under the direction and supervision of the Ward Manager with additional support from the Dementia and Delirium Team. The post holder will contribute to the delivery of patients' individual care under the direction and supervision of the Ward Manager and in conjunction with the Dementia and Delirium Team. They will gather quality collateral history/ personal information from patients, families, carers and other relevant services (eg social worker, care agency). Deliver, monitor and embed DCS care plans for patients with dementia using clinical leadership skills to role model best practice to ward staff. Work closely with dementia service colleagues and contribute to service goals, taking responsibility for collecting and inputting data metrics. Use clinical judgement to feedback to MDT patient progress and assist in setting patient goals that reflect their changing needs. Ensure effective ongoing carer communication/ liaison, including referrals to community resources where indicated (eg carer support organisations). Facilitate the provision of meaningful activities for patients, working closely with band 2s and volunteers. Facilitate supported/ risk-positive discharges. Follow-up telephone calls and where required, admission avoidance activity (community liaison/ follow-up visits).
