This newly created role focuses on enhancing care for dementia patients on acute orthopaedic wards and involves supporting the delivery of individual patient care under the guidance of the Ward Manager and Dementia and Delirium Team. Key responsibilities include gathering patient history, implementing and monitoring care plans, facilitating meaningful activities, coordinating effective communication with caregivers, and contributing to discharge planning. The role entails covering a seven-day service alongside other Dementia Support Workers across three ward areas and involves direct patient care tasks such as personal hygiene assistance and mobility support.
