Dementia Support worker Band 3 Permanent Full Time - 37.5 hours per week Please note this is not a healthcare assistant role. Please read the job description carefully This is a newly created post that will be a part of the Trauma and Orthopaedic Care Group within the Division of Surgery at the Aintree site. The post-holder will support our acute orthopaedic wards to facilitate a high standard of enhanced care for our dementia patients to ensure a robust and holistic hospital admission. The post-holder will contribute to the delivery of patient's individual care under the direction and supervision of the Ward Manager with additional support from the Dementia and Delirium Team. Whilst completing this, the post-holder will assist with all aspects of the patient's care including moving and handling, personal hygiene and dietary needs. The successful candidate will be covering a 7 day service and will work with other Dementia Support Workers to ensure the service is covered across the 3 ward areas Monday - Sunday. **Previous applicants need not apply** Please note this vacancy may close early once sufficient applications have been received. The post holder will contribute to the delivery of patients' individual care under the direction and supervision of the Ward Manager and in conjunction with the Dementia and Delirium Team. They will gather quality collateral history/ personal information from patients, families, carers and other relevant services (eg social worker, care agency). Deliver, monitor and embed DCS care plans for patients with dementia using clinical leadership skills to role model best practice to ward staff. Work closely with dementia service colleagues and contribute to service goals, taking responsibility for collecting and inputting data metrics. Use clinical judgement to feedback to MDT patient progress and assist in setting patient goals that reflect their changing needs. Ensure effective ongoing carer communication/ liaison, including referrals to community resources where indicated (eg carer support organisations). Facilitate the provision of meaningful activities for patients, working closely with band 2s and volunteers. Facilitate supported/ risk-positive discharges. Follow-up telephone calls and where required, admission avoidance activity (community liaison/ follow-up visits). To perform, understand and report patient risk assessments e.g. Nutritional assessment scores,Falls Assessment, Moving and Handling, Pressure Area assessment and Bed Rail riskAssessment. To demonstrate initiative and the ability to recognise/identify changes in a patientscondition/treatment and report promptly to the registered nurse and medical staff as appropriate. To act as a role model for student nurses and colleagues. Report any concerns regarding patients well-being or safety to the nurse in charge. It is the responsibility of any member of staff who becomes aware of either a clinical or non-clinicalincident to escalate immediately to the registered nurse. To understand the principles of the Friends and Family Test/patient experience feedback and howit is used to improve and maintain an excellent patient experience. Assist nursing staff with ward preparation for admission and to ensure the bay areas areappropriately cleaned on patient discharge or transfer.
