Our multidisciplinary team work flexibly to provide a 7-day service, 365 days a year between 8am and 8pm. -We deliver rapid response to people in the community who are experiencing acute episodes of ill-health and require complex management in order to prevent hospital admission and support timely hospital discharges. You will have the responsibility for proactively monitoring and managing patients to ensure the best outcomes. To act as a senior clinical professional within the Rapid Response service, as an integrated health and social care team providing an urgent rapid response to Warrington residents, experiencing a health crisis. The aim is to support people to remain at home rather than be admitted to hospital. Liaising with patients the team and colleagues across the wider health and social care environment including NWAS the postholder will promote, maintain and enhance effective multidisciplinary patient care. To act as an independent autonomous practitioner to provide specialist treatment and advice to patients and staff which will contribute to the assessment, diagnosis, care and treatment of patients at home preventing unnecessary hospital admission. All referrals will be triaged immediately and responded to within two hours, with referrals from NWAS prioritised.
