Bridgewater Community Healthcare NHS Foundation Trust is seeking a Rapid Response Advanced Nurse/AHP to join their Urgent Community Response service in Warrington. The role requires an enthusiastic and skilled individual to work autonomously and as part of a multidisciplinary team, delivering care to patients in their homes to prevent hospital admissions and facilitate timely discharges. The candidate will conduct comprehensive patient assessments, manage acute health crises, perform triage, and implement individualized treatment plans while providing specialist advice and care. This position involves 7-day service availability, and the practitioner will also engage in professional education, clinical audit, and maintain high standards of documentation and communication within the health and social care networks.
