The post-holders will manage and deliver breast surgical wound clinics and work closely with the breast surgeons in the clinical setting. You will be a point of contact for patients who have undergone breast surgery pre- and post-operatively and provide telephone support. Clearly defined pathway, manage and deliver benign diagnosis clinics working closely with the breast surgeons and provide in-patient care to those who have undergone surgery including advice prior to discharge The role also involves supporting the Macmillan Breast Cancer Nursing Services Team who will be present at the time of diagnosis Act as a specialist resource on all aspects of breast wound care to patients, carers and multi-professional team Demonstrate in depth knowledge and skills in breast care Engage in complex conversation and dialogue with patients and carers considering learning needs and Mental Capacity using advanced communication skills Ensure patients understand who/what keyworker is and their responsibilities Follow agreed pathways, provide written and verbal information, refers to other professionals and signpost to other agencies ensuring communication and liaison with other relevant professionals Deliver high quality, clinically effective care, utilising best practice and research, in collaboration with colleagues in everyday practice Use excellent communication skills to promote patient self-management and improve health outcomes and health promotion Act as a role model and ensure effective staff supervision with clinical practice, working with staff and patients in an advisory, teaching and support capacity and maintaining professional competence, authority and credibility Maintain comprehensive patients' records and update regularly in line with Trust and NMC standards for record keeping Ensure clinical governance is incorporated into practice and is able to interpret national policies and guidelines relevant to the speciality Manage a clearly defined caseload of patients Proactively work to minimise/manage risk About us Please see the attached Job Description and Person Specification for more information.
