They will work with members of the team and other health and care providers to share information to jointly plan care and provide a streamlined service. The post holder will be performance managed with clear objectives designed to evidence your success in delivering excellent patient satisfaction and health outcomes, supporting the ADHD Team patient caseload, developing colleagues skills, and lastly supporting patients to avoid unnecessary hospital admissions. The case worker will: Work with the ADHD team co-ordinating care from referral to discharge. Patients will be referred via the electronic referral system and the case co-ordinator will have responsibility, supported by the General Manager, in ensuring all referral documentation requested from the referrer is received (Baseline investigations, ECG, Blood results and questionnaires). Contact the patient to offer a timely, accessible appointment as agreed with the patient, either face to face or virtual. The post-holder will recognise the importance of effective communication within the team and will strive to: Communicate with patients in a welcoming way, which is non-judgemental and respects patients feelings, circumstances and rights. Utilise and demonstrate sensitive communication styles to ensure patients are fully informed of reasons for contact when contacting them for example, on welfare calls Communicate highly sensitive patient information and progress effectively with other team members and outside relevant health and social care providers, as appropriate Ensure all documented notes are collated and comprehensive About us The case worker will: Work with the ADHD team co-ordinating care from referral to discharge. If any concerns regarding priority of appointment timing this can be discussed with the General Manager or a Clinician. Call the patient on a regular basis, to carry out a welfare call general well-being, medication compliance, any concerns. Arrange follow up appointments with the Non-Medical Prescriber (Nurse or Pharmacist). Ensure, after each appointment that the Patient and registered GP receives a copy of the consultation / care plan. Ensure, when the patient is discharged back to their registered GP as part of the shared care agreement, that all documentation and the individualised care plan is completed and shared. Where identified, signpost the patient to appropriate support services, which may include self-help resources, Pharmacists, Talking Therapy Services, Voluntary Care Services amongst others. Support the individual / carers in completion of relevant documentation needed EG. Benefit claims, supporting letters for employers, educational establishments etc Attend multidisciplinary and team meetings as needed.
