North House Surgery is a friendly Practice with an approximate list size of 13500 patients. We are a busy Practice situated in the heart of Crook. We are looking forward to welcoming a Practice Nurse to expand and enhance our team, working part time. As well as general practice nursing duties (including, but not exclusive to, women's health and screening, vaccinations, injections and health promotion), this post has a substantial focus on delivering effective individualised care to patients. The successful candidate will be required to work within the professional boundaries of a qualified nurse and provide an extremely high standard of nursing care for patients registered at the practice. The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice To develop, implement and embed health promotion and wellbeing programmes To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF To prescribe medication for patients in accordance with PGDs and own scope of practice Provide wound management/wound care To administer travel vaccinations To request pathology services and process pathology results as required To maintain accurate clinical records in conjunction with extant legislation To ensure read codes are used effectively by all members of the nursing team To ensure compliance with the practice chaperone policy To assist GPs with minor surgery when required To prioritise health issues and intervene appropriately To support the team in dealing with clinical emergencies To recognise, assess and refer patients presenting with mental health needs To administer adult and child vaccinations Women's Health And Screening About us To work within the professional boundaries of a qualified nurse. To provide and maintain an extremely high standard of nursing care for patients registered at the practice as well as providing nursing assistance to the GP/Nurse Practitioner Team. Responsible for working as part of the primary care team providing a comprehensive range of general nursing and clinical duties. The Practice Nurse will be responsible for providing a range of treatment room procedures including wound care, removal of sutures, injections, immunisations, health checks and cervical screening. North House Surgery will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.
