Patients who are due chemotherapy or SACT in the following 2 - 3 days are clinically assessed over the telephone and their blood results analysed. The SACT can then be authorised. The VAP service is not just a pre SACT assessment clinic, we manage the patients' cancer pathway and as a result we become part of their journey. We maintain continuity of care and as a result we develop a relationship with our patients as we often assess them before each SACT for months at a time. The VAP clinic currently works with the following teams assessing patients on suitable regimens from the following cancer sites Colorectal / Breast / Urology / Gynae / Skin. The post holder is responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care, participating and contributing to the wider clinical governance agenda including service developments and flexible working to meet the needs of patients. The working week hours can be flexible. This job can be part time / full time/ job share. We can consider compressed weekly hours. The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply. To work with patients and their carers to determine in partnershipthe assessment of patient needs. Working collaboratively with themulti disciplinary/multi agency team. To liaise and provide guidance on your area of practice to inpatientand outpatient on the care of patients when requested.
