This is an exciting opportunity for a motivated and competent Therapist to join our established team in providing assessments and interventions for patients living with long term conditions experiencing symptoms of breathlessness, fatigue and associated anxiety. You will also have an opportunity to participate in on-going research, teaching, innovative developments and service evaluation. You will manage your own daily workload of patients, visiting patients in their own homes, in Outpatient Clinics or occasionally on the Wards. The team work remotely, but have the option to be based in the office, if preferred. You will be working in an established Palliative Care Team that includes Occupational Therapy, Physiotherapy, Doctors, a Clinical Psychologist and Clinical Nurse Specialists. You will have access to the support and resources you require in order to succeed in this post. To perform specialist Physiotherapy / Occupational Therapy assessment of patients living with breathlessness and associated anxiety and fatigue. To develop and deliver individualized treatment programmes and to work with patients and families to develop self management skills. To undertake patient assessment in the home, out-patient or telephone / video setting. To liaise with a range of services and health care professionals in primary and secondary care and to negotiate appropriate services and interventions for BIS patients. To maintain records on EPIC as an autonomous practitioner in line with CSP / RCOT and Trust standards. Contribute to the development of and adhere to BIS / Palliative Care record keeping protocols and provide effective written communication with all relevant services/colleagues. To educate and train Occupational Therapy / Physiotherapy colleagues and multidisciplinary teams in primary and secondary care regarding the AHP role in BIS. To undertake evidence-based practice and contribute to BIS audit and research projects. To contribute to dissemination of these findings within the Trust and broader (such as academic meetings and conferences). To communicate effectively with patients, relatives, other AHPs and the multidisciplinary team to ensure a high standard of patient care and service provision.
