The ideal candidate will have an interest in working within a busy and dynamic Community Mental Health Service. The typical job roles and responsibilities will include: Undertaking urgent / routine initial assessments on behalf of the team. Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as CRHTT, Adult Social Care and third sector organisations). Supporting and managing a caseload undertaking face to face visits in the community setting including home visits. Working with service users and their families / carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal / external stakeholders. Working alongside Lead Care Professionals and supporting those currently under the Care Programme Approach. Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm). Medication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication management.
