Band 6: £37,338 to £44,962 a year gross per annum  FIXED TERM  GOOD SALARY 

The ideal candidate will have an interest in working within a busy and dynamic Community Mental Health Service.

We currently have a fixed term Band 6 Community Mental Health Nurse post within Central Adult CMHT open to applications from Registered Mental Health Nurses. The team is based in Stowmarket and covers a large geographical rural area. We are a recovery focussed, multi-disciplinary team providing an effective, supportive, and professional service for our Services users and Carers.

You will be a qualified professional with experience of supporting people and their families, preferably in the community. You will demonstrate a strong understanding of your role as a Care Coordinator / Assessor and join our dedicated multidisciplinary team working closely with a range of partners to provide appropriate community interventions for those individuals referred to the team.

The ideal candidate will have an interest in working within a busy and dynamic Community Mental Health Service. The typical job roles and responsibilities will include:

Undertaking urgent / routine initial assessments on behalf of the team.

Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as CRHTT, Adult Social Care and third sector organisations).

Supporting and managing a caseload undertaking face to face visits in the community setting including home visits.

Working with service users and their families / carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal / external stakeholders.

Working alongside Lead Care Professionals and supporting those currently under the Care Programme Approach.

Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm).

Medication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication management.

Essential criteria

  • Nursing degree or equivalent
  • Mentorship qualification or ENB 998. Willingness to train to become Sign-off Mentor
  • Higher degree or post graduate certificate in area of clinical / therapeutic practice


Desirable criteria

  • Good to great management course, or equivalent


Experience


Essential criteria

  • Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
  • Demonstrate effective risk assessment and risk management experience
  • Experience in the delivery of clinical interventions
  • Experience in working in team delivering person centred treatment packages


Desirable criteria

  • Experience in delivering group-based interventions
  • Experience in supporting service development


Knowledge


Essential criteria

  • Knowledge of medication and the monitoring of its effects
  • Knowledge of psychosocial interventions
  • An understanding of the policies and guidelines both local and national e.g., National Institute for Clinical Excellence guidelines (Government initiative)

  • Knowledge of health promotion
