Our UCR team also supports the delivery of our Virtual Wards (Hospital@Home) service that facilitates early supported discharge from hospital as well as ongoing admission avoidance clinical management. As a Band 7 Urgent Community Response Clinical Lead (nurse/paramedic/AHP) , you will play a crucial role in assessing, coordinating, and delivering high-quality, patient-centred care to individuals experiencing acute episodes of illness. You will lead a multidisciplinary team to respond swiftly to urgent care needs, providing short-term interventions, and working closely with GPs, hospitals, and other community services. Key Responsibilities: Lead and support a team of healthcare professionals to deliver timely and effective urgent care services. Undertake advanced clinical assessments to identify and manage patients' urgent health needs. Develop care plans to support patients' recovery and prevent unnecessary hospital admissions. Work collaboratively with other services and agencies to ensure seamless, integrated care. Provide clinical leadership, mentoring, and support to junior staff and colleagues.
