Your role will involve providing generalist clinical care and advice to patients and the Integrated Urgent Care Team. Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check. Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position. To provide highly specialist, high-quality, person-centred care that always considers people's safety, privacy, and dignity, using advanced clinical knowledge that is evidence-based. As part of a multi-professional team, the post holder will practice autonomously, while being accountable and self-directed in line with the relevant code of professional conduct. To plan and manage complete episodes of care; undertake independent, comprehensive assessment and management of service users, incorporating pharmacological considerations and making complex clinical decisions regarding service user management and clinical outcomes. To provide expert clinical advice, leadership, and support, ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis, and advanced decision-making skills.To lead as a driver for change, monitoring and improving standards through supervision, evidence-based practice, clinical audit, research, and education. To promote and demonstrate best practice by integrating evidence into practice.To work strategically, contributing to the planning and development of integrated services.The post holder will be accountable for the direct delivery of autonomous service user care.
