The Mindworks Alliance is a partnership between traditional CAMHS Services and third sector/voluntary agencies working to meet the emotional and mental health needs of CYP in Surrey and now NE Hants. This can be a developmental post so if you are ready for a Band 6 role we are interested in developing you further in to a Band 7 role within 2 years as part of your career pathway. Our team provide a 7-day week Crisis Intervention Service, offering discovery conversations to CYP & their families/carers presenting with emotional and mental health crisis in local acute hospitals and community bases. Our operational hours are 9am 5pm Monday-Sunday including Bank Holidays. Unsocial and weekend shifts will be expected and we use the e-roster system to ensure you are aware of your shifts. Your main base will be Aldershot Centre for Health, though you will be expected to in reach to Frimley Park Hospital and homes of CYP. The successful candidate will hold an appropriate professional registration with NMC or HCPC or SW England. Experience and competence in working with vulnerable people in crisis is essential. Working with young people in crisis is desirable. The main duties within this role are leading and supporting assessment, risk and care planning alongside CYP, their family/carers and the acute staff to safely facilitate discharge with follow up care planned in the community within 7 days. Where appropriate you will offer up to 6 follow up sessions with the CYP and ensure referral and signposting to appropriate services for longer term follow up is put in place and supportive discharge/transfer facilitated. Supporting junior staff to provide high quality care, sharing learning and developing skills through supervision and training while maintaining KPI's and collecting data to support the service growth and development are all part of this exciting role. These posts offer structured career progression with recognised training packages including CPD modules in assessment and risk assessment of adolescents and DBT-A. About us Band 6 Key Responsibilities You will work closely within the Mindworks Alliance during office hours, responding to crisis referrals of CYP You will work as an autonomous practitioner, undertaking clinical assessment of CYP who may have a complex co-morbid mental health needs and present in crisis, working alongside medical, nursing and other professional colleagues to ensure that the most effective care and treatment is provided resulting in safe, timely discharge from acute or T4 inpatient care. To co-ordinate and liaise with Paediatric ward and SABP Core 13 (Paediatric Liaison Service) and the other CIS team members to facilitate timely discharge from hospital and follow up in the community. Work closely with the Emergency Departments, the Adult Psychiatric Liaison Service, Mental Health Crisis Teams, On Call Duty Psychiatrist, On Call Managers, Core 13 (Paediatric Liaison Mental Health Nurses), ensuring CYP are seen in a timely manner and risk is assessed and a management plan is developed. To manage own caseload from referral to discharge/transfer, holding medium to high risk cases and ensure timely and accurate documentation is completed. Ensure that outcome measures for Crisis Intervention are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based. Be responsible for the initial assessment and subsequent facilitation of referral to appropriate services once the crisis has become manageable. Report to and advise community referrer on assessment and treatment outcomes and respond to requests for and provide formal reports for other agencies. Represent the Crisis Intervention Service at professional/network meetings to offer advice/consultation on complex cases who are in crisis. Develop and implement systems to monitor and evaluate the service To undertake relevant CPD to improve assessment and treatment skills regarding crisis intervention.
