Follow-up telephone reviews, and at times face to face assessments, for patients leaving the acute setting who are transferred to a community hospital. To assess, diagnose and treat patients with communication and swallowing disorders. Regular triage and prioritization of caseloads. To work as part of multi professional team in the management of acutely unwell patients on the medical and surgical wards and the acute stroke unit. To actively contribute to service developments and incorporate these into core services, in areas of communication and swallowing.
