This will provide an exciting opportunity to gain experience of working within the South Uttlesford Primary Care Network (PCN), providing a specialised dietetic resource for patients within the PCN. There are 5 GP practices within the PCN with a registered population of circa 51,500. The post holder will be part of the PCN Primary Care Aligned Team (PACT) and will work closely with the other members of the multidisciplinary PACT to support the management of patients with long term conditions. The post holder will be responsible for providing leadership and dietetic clinical expertise/advice and guidance in partnership with the multidisciplinary PACT. The post holder will fully integrate within the PCN team and become a significant source of expert dietetic advice in the network. The post holder will be clinically responsible for the dietetic assessment and/or treatment of patients with chronic diseases as well as improving care to groups with particular health needs such as diabetes, chronic pain and menopause for example. About us The post holder will act as a dietetic resource raising nutritional awareness by providing training to the wider multidisciplinary PACT and promoting the benefits of dietetic services with respect to a wide range of diet related conditions within the PCN population. This will include developing and delivering group education sessions and workshops. The post holder will also be part of the EPUT (Essex Partnership University NHS Foundation Trust ) community dietetic team receiving supervision, peer support and CPD . Full training in Systmone and Emis systems will be provided. Prevention, early identification of long term conditions, and the management of those long term conditions once identified is key to achieving the PCN ambition of keeping people well for longer and improving their outcomes.
