The Specialist Dietitian for Chronic Disease Management in South Uttlesford, with the Essex Partnership University NHS Foundation Trust, is a fixed-term position covering maternity leave for one year. The role involves working within the South Uttlesford Primary Care Network (PCN), which serves approximately 51,500 patients across five GP practices. The dietitian will collaborate with the multidisciplinary Primary Care Aligned Team (PACT) to deliver specialized dietetic support for patients with chronic conditions such as diabetes, chronic pain, and menopause, focusing on improving patient care and nutritional awareness.

The post holder will provide clinical expertise, conduct dietetic assessments, and lead educational initiatives aimed at both patients and the PACT team to promote healthy dietary practices. Responsibilities include developing and delivering group workshops and training sessions while receiving supervision and support from the EPUT community dietetic team. A valid driver’s license is required for this role.
