Waltham Forest Rapid Response Team is 24 hours 7days a week multidisciplinary service, providing a crisis response service with a two hour response to referrals from various health and social care organisations and GPs. The service aims to: avoid unnecessary hospital admissions; support hospital discharges; provide unplanned nursing/therapy care 24/7 including crisis management for patients at the end of their life and pick up falls in their own home. You will Work collaboratively with health and social care professionals across the borough and Integrated Care System. Liaise with other teams/organisation to plan joint care and safe discharges Support the delivery of complex nursing care with other multi-disciplinary agencies. Delivering aspects of community based interventions across nursing, social and therapy specialities. Be able to develop and extend your skills into areas where you may have less knowledge and/or experience. A willingness to learn is therefore key. Work collaboratively and carryout joint visits with MDT colleagues within the team to promote holistic person-centred assessment and approach to care Participation in clinical supervision Be supportive of colleagues Contribute towards and carryout audits within the service Report incidents appropriately using trust and national frameworks and help support any lessons learnt within the service. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes.
