Waltham Forest Adults Community Health Services – Unplanned Care



37.5 hours per week


Are you a caring and confident individual with enthusiasm and a wish to deliverer person cantered care in the community, alongside a flexible welcoming multidisciplinary team

Working as part of the Waltham Forest Rapid Response team you will experience a rewarding and varied day, everyday.

Waltham Forest Rapid Response Team is 24 hours 7days a week multidisciplinary service, providing a crisis response service with a two hour response to referrals from various health and social care organisations and GPs.

The service aims to: avoid unnecessary hospital admissions; support hospital discharges; provide unplanned nursing/therapy care 24/7 including crisis management for patients at the end of their life and pick up falls in their own home.

You will

  • Work collaboratively with health and social care professionals across the borough and Integrated Care System.
  • Liaise with other teams/organisation to plan joint care and safe discharges
  • Support the delivery of complex nursing care with other multi-disciplinary agencies.
  • Delivering aspects of community based interventions across nursing, social and therapy specialities.
  • Be able to develop and extend your skills into areas where you may have less knowledge and/or experience. A willingness to learn is therefore key.
  • Work collaboratively and carryout joint visits with MDT colleagues within the team to promote holistic person-centred assessment and approach to care
  • Participation in clinical supervision
  • Be supportive of colleagues
  • Contribute towards and carryout audits within the service
  • Report incidents appropriately using trust and national frameworks and help support any lessons learnt within the service.

Applicants are required to evidence a full UK driving licence and the ability to travel to a variety of sites across Waltham Forest. (This criteria will be reasonably adjusted to meet the requirements of a disabled applicant)

Starting with NELFT


NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.


COVID-19 Vaccination


We continue to encourage all staff to ensure that they have been double vaccinated and received their booster. We recognise that taking the vaccine provides the best defence against COVID 19 for our patients, our staff and their families.


Probationary Period


This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).


High Cost Area Supplement – London


This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).
