The successful applicant will work directly and semi-autonomously supporting persons living with neuromuscular conditions in Surrey and working very closely with the specialist neuromuscular team at the Atkinson Morley Regional Neurosciences Centre at St George's Hospital. Please note that this role will involve some travel within Surrey and a driving licence and access to their own car would be of great benefit. They will be able to support patients to ensure appropriate support is offered between specialist centres, local general neurology services and community teams. This is essential to reducing unplanned admissions, reducing the morbidity and mortality of people living with neuromuscular disease and achieving cost savings, which is a crucial part of this role. The focus of this role will be to provide holistic support and advice to adults and young people living with a neuromuscular condition across Surrey. The post holder will work closely with individuals living with a neuromuscular condition and their families to co-ordinate clinical care and improve overall quality of life. They will work relatively autonomously in the community for two to three days a week. Two to three days a week will be spent working as part of the neuromuscular multi-disciplinary team at St Georges Hospital, which will have responsibility for training, supervision and clinical governance.
