Job summary

This is an exciting 12-month fixed term contract, suitable for a therapist or nurse, to work seamlessly with patients across specialist community care environments in Surrey and the regional specialist neuromuscular unit at St George's Hospital. The role will support the care of patients with neuromuscular disorders - a spectrum of individually rare diseases affecting the function of nerves and muscles, such as muscular dystrophies or neuropathies which can cause significant neurological disability. These conditions often require multidisciplinary care between doctors and other professionals working at multiple sites. This role will act to provide coordination, advice and support for persons living with a neuromuscular condition. This is an excellent opportunity to work in a specialist service and develop case management skills.

The successful applicant will work directly and semi-autonomously supporting persons living with neuromuscular conditions in Surrey and working very closely with the specialist neuromuscular team at the Atkinson Morley Regional Neurosciences Centre at St George's Hospital.

Please note that this role will involve some travel within Surrey and a driving licence and access to their own car would be of great benefit. Travel costs incurred as part of the role will be reimbursed.

The post holder will work closely with individuals living with a neuromuscular condition and their families to co-ordinate clinical care and improve overall quality of life. They will work relatively autonomously in the community for two to three days a week. Two to three days a week will be spent working as part of the neuromuscular multi-disciplinary team at St Georges Hospital, which will have responsibility for training, supervision and clinical governance.
