ML Personalised Healthcare Commissioning (PHC) team are looking for a qualified Nurse to work with our team based in Staffordshire. The post holder will play a key role in supporting the Personalised Healthcare Commissioning service across the region. Working within the remit of the National Framework for Continuing Healthcare and other NHS funding streams, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service. The post holder will be responsible for assessing patients, ensuring The National Framework for NHS Continuing Healthcare Funded-Nursing Care and S117 entitlement is implemented and complied with, according to the principles and processes. The post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.
