ML Personalised Healthcare Commissioning (PHC) team are looking for a qualified Nurse to work with our team based in Staffordshire.
The post holder will play a key role in supporting the Personalised Healthcare Commissioning service across the region. Working within the remit of the National Framework for Continuing Healthcare and other NHS funding streams, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.
The post holder will be responsible for assessing patients, ensuring The National Framework for NHS Continuing Healthcare Funded-Nursing Care and S117 entitlement is implemented and complied with, according to the principles and processes.
The post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.

Currently we are looking for a motivated and experienced CHC nursewho can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.
Our hours of work are Monday-Friday, 9am -5pm with the opportunity for agile working. ML is committed to the personal and professional development of their staff and offers a wide selection of learning and development opportunities.
Currently the post is agile working with a requirement to attend the office base as and when requested. All assessments will be face to face, a vehicle for the purpose of business miles is essential for this role.
