Role Overview This position offers a unique chance to work alongside a highly motivated and skilled outreach team and in partnership with local services dedicated to providing care and support for babies, children, young people, and their families. You will play a key role in providing a comprehensive social work service to babies, children and young people with life-limiting conditions and their families, focusing on the co-ordination, development and delivery of service provision designed to assist with the issues related to the child or young person’s condition and the impact it has upon the wider family.
