Main duties of the job To participate in the development of a range of medicines management activities at ward level including medication history taking and medicines reconciliation to inpatient units including Psychiatric Intensive Care (PICU) wards at Tameside and Stockport, and Radcliffe Place (Learning Disability unit) in Bury. To coordinate the supply of medication to ensure good medicines management practices. To act as a key member of the multidisciplinary ward team and the wider pharmacy team providing medicines management advice to healthcare professionals and counselling to patients and carers. To participate in local and national audits and provide information as required.
