You will identify immediate health needs including, any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires close joint working with other in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education, etc. You will also; Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections Deliver continuity of care across health and social care agencies from courts, to custody and community whilst improving the communication between the above. Inspire the continuous improvement of MDT working across HMP and healthcare teams. Hours The site currently offers Monday to Friday with flexibility on hours. This position is full time working 37.5 hours per week. Salary Youll receive an annual salary up to £44,000 per annum - depending on experience. How will we support you?
