Depending on experience Up to £44,000 per annum  PERMANENT 

As a Long Term Conditions Nurse you will be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings. You will identify immediate health needs including, any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing

A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires close joint working with other in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education, etc.

You will also;


Provide
proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections


Deliver
continuity of care across health and social care agencies from courts, to custody and community whilst improving the communication between the above.


Inspire
the

continuous improvement of MDT working across HMP and healthcare teams.




Promote
patient confidence about the management of their condition and fosters patient responsibility to make appropriate healthy choices


Share
ways to reduce demand into other health and HMP resources (e.g. escorts and transfers to primary and secondary care) whilst improving access to and utilisation of health or social care resources

category C

 Prison located in London Borough of Lambeth, in inner-South London

Hours

The site currently offers Monday to Friday with flexibility on hours.

This position is full time working 37.5 hours per week.  


Salary

 Youll receive an annual salary up to £44,000 per annum - depending on experience.

As a Long Terms Conditions Nurse, your responsibilities will vary. You will

  • Lead innovative nursing practice in the management of long-term conditions for a specific caseload of patients. This includes the assessment of need, delivery, and evaluation of care interventions and management plans.
  • Proactively manage frequent and high demand users of health services using long term condition strategies in line with the Department of Health standards.
  • Provide professional support and guidance for the nursing team and ensure resources are utilised effectively for the day to day management of patients with long term conditions.
