Our service supports patients to return home from hospital sooner, continuing healthcare assessment, rehabilitation, reablement and recovery within the patient’s home environment. We also provide an urgent crisis response, supporting patients during an acute episode of illness and or injury, preventing unnecessary hospital admissions. You will work closely with a team of nurses, physiotherapists, occupational therapists and pharmacists to support patients’ individual needs, promoting patient health and wellbeing. Please Note You must be able to commit to our full time, 4 week long Healthcare Academy Programme for your first 4 weeks with our Trust We are unable to provide Sponsorship for this role. You will undertake specific duties delegated by the nursing, physiotherapy and occupational therapy team and will be responsible for providing holistic, person centred, compassionate health care to patient’s 18 years and over, supporting rehabilitation, reablement and recovery within the comfort of the patient's home. We are looking for people who are kind, compassionate, creative, problem-solvers with excellent communication skills to join our expanding team. The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. We provide a seven day service, between the hours of 8am - 9pm; therefore, successful candidates will take part in a rota that covers the hours of the service; however, we do offer a flexible and varied shift pattern.
