Morden PCN encompasses Morden Hall Medical Centre, Central Medical Centre, Stonecot Surgery and Ravensbury Park Medical Practice and covers a total registered list of around 39,000 patients. They will have a primary practice base at Central Road Medical Practice . The PCN practices strive to provide high quality patient care and have a strong base of education and training. The main role of the successful applicant will be to proactively communicate information between patients, doctors and professionals. This will include assisting the clinical teams with administrative functions within the practices. There will be peer-learning opportunities for the care coordinators across the PCN and also training and development opportunities. This role requires excellent communication, IT and organisational skills including an ability to prioritise, multitask and work to deadlines.
