Morden PCN Care CO-ORDINATOR

RESPONSIBLE TO:

Morden PCN - Practice Manager and Clinical Director

JOB PURPOSE:

To work across the 4 practices that make up Morden PCN

Organise and work with Video consultation groups

E-hub monitor data submitted by patients online and ensure it is dealt with appropriately

To proactively communicate information between patients, doctors and professionals

To assist the clinical teams with administrative functions within the practices

To assist the doctors and other clinicians in the provision of a high-quality service across the practices.

Administer information received via the central e-hub for the PCN and ensure it is dealt with appropriately and efficiently working to protocols.

Organise and facilitate video group consultations (training provided).

Seeking patient information and results from hospitals and other organisations.

Answering general enquiries, making new and follow up appointments.

Giving results to patients under Clinical direction.

Contacting hospitals to follow up referrals.

Liaise with hospitals for updates on patients or advise of any relevant changes in patients condition as directed by clinicians.

Completing and submitting quality alerts as appropriate.

Dealing with requests for information (e.g Merton MASH).

Organising & attendance at MDT meetings.

Taking of notes at MDT meetings & preparation of associated minutes.

Update Coordinate My Careregister.

Booking of follow up appointments for patients that are requested by clinicians.

Supporting reception team in dealing with transport request

Reviewing, filtering and flagging information on Docman requiring urgent action to clinicians.

Contacting/chasing patients as requested by the Clinicians.

Pro-actively ensuring that booked appointments are appropriate & with the correct clinician

Overseeing reports and prompting Doctors when urgent. Preparation of eGPRs where necessary.

Look up results on the EPR system or chase with hospital/lab

Assisting clinicians with Care Plans. Coordinating & actioning admin aspects of care plans.

Undertaking quality improvement and change in practice. Assist with audits as required

Understanding the importance of medical terminology, data and coding

Record keeping with use of appropriate coding.

Understand vulnerable groups and long term illness. Use of patient information resources

To work as a positive and active member within the Practice and PCN teams..

Take part in an annual appraisal at which your performance will be reviewed and your training needs discussed.

Attend relevant meetings as required including staff meetings. You may be requested to attend other meetings at the request of the medical or management team.

Supporting, supervising & mentoring others

Participate in peer learning with other Care Coordinators within Morden PCN

Morden Primary Care Network is looking to recruit a Care Coordinator to join one of our practice teams. Morden PCN encompasses Morden Hall Medical Centre, Central Medical Centre, Stonecot Surgery and Ravensbury Park Medical Practice and covers a total registered list of around 39,000 patients. They will have a primary practice base at Central Road Medical Practice . The PCN practices strive to provide high quality patient care and have a strong base of education and training.

The main role of the successful applicant will be to proactively communicate information between patients, doctors and professionals. This will include assisting the clinical teams with administrative functions within the practices. There will be peer-learning opportunities for the care coordinators across the PCN and also training and development opportunities. This role requires excellent communication, IT and organisational skills including an ability to prioritise, multitask and work to deadlines. The role will be a key member of the base practice teams and the successful applicant would need to work to a high standard. Experience of working in a Primary Care setting would be desirable.
