Work in partnership with the Macmillan Nursing Team to provide patient coordination and care for breast cancer patients who are low risk i.e. non-complex care needs. Work in conjunction with the Macmillan Breast Nursing Team to provide coordination of high quality patient care through on-going telephone/ face to face assessment and proactive identification of needs using basic knowledge, approved tools and procedures. Collaborate and communicating with the Macmillan Breast Nursing Team , and wider health care teams to ensure the provision of appropriate services at the right time and in the right place. Be a single point of contact for patients, carers and the healthcare team Care Coordination is the joining up of services, coordination, information and communication between care givers, treatment providers, those living with and beyond cancer and their families that creates a seamless experience of care (NHS Improvements, 2011). The purpose of this role is to support the Macmillan Breast Nursing Team in designing and implementing The Recovery Package and Open Access Follow Up for colorectal cancer patients after completion of treatment. The post will require the coordination of appropriate services to maximise the delivery of the Recovery Package (RP) and Open Access Follow Up (OAFU), thereby improving the quality and experience for patients and their carers.
