Advanced communication, problem-solving anddecision-making skills will be used to initial assess and provide on-goingpatient care. Working within local andnationally agreed protocols and guidelines, the Specialist Nurse exercisesindependent judgement to assess, investigate, diagnose, plan, implement andevaluate the clinical care and management of community patients. The post holder will be part of a team that leadsnon-medical models of care across primary and community-based settings,including care homes and within community teams, in collaboration withmulti-professional and multi organisational colleagues.
