Job summary

Work autonomously as a Community Frailty SpecialistNurse, across traditional boundaries, within the Frailty Team and with PrimaryCare Networks providing a high level of expertise to ensure patient-centredclinical care. They will act as a highlyspecialised role model for advanced practice, working closely with themultidisciplinary team and other key stakeholders.

Exercise clinical expertise, levels of judgement,discretion and decision making whilst undertaking the skills of assessment,examination to provide a diagnosis, formulate management plans both reactiveand proactive within an agreed scope of practice.

Evaluate investigations and revise treatment plansaccording to patient need and assess impact and outcome.

Undertake advanced clinical assessment andexamination, including but not limited to:

  • Clinical datainterpretation, investigation and treatment.
  • Ability to developadvanced comprehensive care planning and reviewing and assessment ofinterventions.
  • Diagnosis anddisease management and delivery of expert clinical interventions andtreatments.
  • Non-medicalprescribing.
  • Assess and treatvaried clinical cases, having advanced skills to assess the patient with adifferentiated or undifferentiated clinical diagnosis
  • Workcollaboratively within the multi-professional team and support the educationand development of both nursing, medical and allied-health professionals.

Main duties of the job

The Community Frailty Specialist Nurse will workcollaboratively with their respective Primary Care Network in Sunderland andthe wider Neighbourhood Team to meet the needs of patients.

Advanced communication, problem-solving anddecision-making skills will be used to initial assess and provide on-goingpatient care. Working within local andnationally agreed protocols and guidelines, the Specialist Nurse exercisesindependent judgement to assess, investigate, diagnose, plan, implement andevaluate the clinical care and management of community patients.

The post holder will be part of a team that leadsnon-medical models of care across primary and community-based settings,including care homes and within community teams, in collaboration withmulti-professional and multi organisational colleagues. They will also play a crucial role instepping up, stepping down and proactive care through timely assessment,referral, responsive intervention and care planning.

Therewill be dedicated time for continuous professional development.
