Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population. Fell Cottage Surgery will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis. About us Fell Cottage surgery islocated in the heart of Low Fell in Gateshead, our surgery is a listed buildingwhich was once a residential property. We are lucky to led by four GP partnersand two salaried GPs in surgery and supported by a great nursing team consistingof one Lead Nurse, one Practice Nurse and two Health Care Assistants. We are atraining practice supporting GP trainees, Physician Associates, Career StartNurse and Training Nurse Associates. Our Admin team of 11 staff are supportedby our Office Manager and our Secretary. Our surgery benefits from having ourown Patient Care Coordinator who works directly with our patients. We also workalongside our extended team of Additional Role Staff which includesPharmacists, Counsellors, Mental Health Practitioners, Social Prescribers,Midwife and Health Visitors. Job description Job responsibilities Primary key responsibilities The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels: To act as support to the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the practice manager To support the production of the IPC annual statement in conjunction with the practice manager To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice To develop, implement and embed health promotion and wellbeing programmes To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF As required, to diagnose and manage acute and chronic conditions with support from the clinical team.
