Fell Cottage Surgery are looking for a motivated and enthusiastic Practice Nurse to join our developing, friendly, patient focused practice team.

The Practice nurse will be responsible for a number if clinical areas such as infection prevention and control, health promotion, chronic disease management, immunisations, cervical screening, as well as actively supporting the practice team in delivering clinical policy and procedures.

It would be beneficial to hold a good understanding of Primary Care and Qof.

Main duties of the job

The Practice Nurse will be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.

Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

Fell Cottage Surgery will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

We are atraining practice supporting GP trainees, Physician Associates, Career StartNurse and Training Nurse Associates. Our Admin team of 11 staff are supportedby our Office Manager and our Secretary. Our surgery benefits from having ourown Patient Care Coordinator who works directly with our patients. We also workalongside our extended team of Additional Role Staff which includesPharmacists, Counsellors, Mental Health Practitioners, Social Prescribers,Midwife and Health Visitors.
