Rate of Pay : £15.43 - £19.30 per hour Summary: Assisting adults (with learning disabilities) to resolve major difficulties in their lives and to live as independently as possible, supporting those we work with to achieve their goals Providing a direct service to adults with learning disabilities and their families/carers in line with the relevant legislation, council policy and procedures Working positively with other agencies such as health, third sector, housing and police where needed, to ensure an appropriate multi agency approach and to support people to speak for themselves Responsibilities: To work in partnership with individuals, and involved networks to help them identify strengths, needs and outcomes To support individuals to maximise their independence and coping skills To undertake assessments including wellbeing assessments and risk assessments with individuals and their networks, whilst considering outcomes, available resources, policy and legislation To undertake non-complex capacity assessments To make recommendations regarding packages of care to meet identified needs, considering the flexible use of all available resources to meet identified needs Qualifications: GSCEs including maths & English, Post GCSE level Qualification Experience: Experience of working with people with learning disabilities and autism Understanding of the social model of disability Please note: You should be available to work immediately or at a short notice.
