The post holder will need to travel across sites. The post holder will complete a range of activities to assist with the completion of physical health checks in service users with Severe Mental Illness (SMI). The post holder will be required to use their initiative when working independently in the community, in both a primary and secondary care setting. To contribute to the delivery of a physical health assessment, care plans under the direct supervision of the practitioner. To report any concerns or identify any physical health care issues to the MDT, Nurse Practitioner and GP. To report to the qualified staff any observations made whilst providing care to service users activities and leisure, providing details of mental state and risk factors. To work with service users on an individual and group basis as outlined in the care plan and as directed by the appropriate named lead professional.
